1. Contact Creative Solutions for Hope (CSH): Call or e-mail our Intake Coordinator to discuss your child’s needs and our services. You may use the Enrollment form below as well.
2. Intake Packet: Once you have decided to start services, our service agreement packet will be sent to you that contains information on our practice. Some of the information requested includes a list of prior treatments, current school, and other background information that will be needed in order to complete a treatment plan.
3. Assessment Process: The Behavior Analyst will assess the skills that your child exhibits so that we can measure the progress made throughout the program. CSH utilizes both standardized and curriculum based assessments to establish baseline skills. The assessment processes includes: parent interviews, observations, and direct assessments. Children that are exhibiting maladaptive behaviors will also have a Functional Behavior Assessment (FBA) to address all areas of need. Once the assessment is complete, a report will be submitted to the appropriate funding source who can then authorize the commencement of services.
4. Starting Services: Once the funding source has provided authorizations to commence services, you will be contacted by our scheduling department to begin the scheduling portion of services. Each team consists of a Clinical/Case Supervisor and one or more Behavior Interventionists depending on the intensity of services.
